Why Nepal keeps winning
Kathmandu and Pokhara are the cheapest pair on this list by a wide margin, and it's not close. Visa-on-arrival costs next to nothing, the flight from Karachi or Lahore is short, and on-ground costs in Nepal run well below Southeast Asia. Seven of the ten destinations above run on eVisa: apply online, get approval by email, no embassy queue. Processing is typically a few days to two weeks depending on the country, so the only real planning constraint is applying early enough, not the visa itself. Turkey and the UAE tend to have the fastest turnaround; budget more lead time for Myanmar.
What doesn't make this list, and why
Countries that require an embassy visit (most of Schengen, the UK, the US) are excluded here entirely, even the ones that would technically fit the budget. The whole point of this list is trips you can actually book without a consulate appointment eating a week of your time. If you're willing to do the embassy route, that's a different list; this one is for "I want to book this weekend."
Going with more than one person, or a longer trip
Every number above is for a solo traveller, five nights, budget style. Add a second traveller and lodging scales sub-linearly (a double room isn't 2× a single), but flights and visa fees are per person, so the total moves up meaningfully, not proportionally.
